ACE American Isurance Company Accidental Death and Dismemberment (AD&D) and Common Carrier (CC) Insurance or Scheduled Airline (SA)
Club members are covered when they are officially enrolled in your account. An insurance certificate is in the member packet. If a club member wants to file a claim, call The Sisk Company at 1-800-899-7175. We will mail a claim packet to the sponsoring organization.

  • Coverage includes various amounts of Accidental Death & Dismemberment Insurance and Common Carrier Insurance. Common Carrier coverage applies if your customer dies in an accident as a passenger on a vehicle licensed to carry passengers for a fare. Scheduled Airline coverage applies to commercial air travel only.
  • Nationally, approximately 100,000 accidental deaths occur per year.

An application is included in the member packet or available online. Club members simply complete the application and return it to the address indicated on the flyer. The club member will be mailed a packet of information including a form to enroll in the credit file monitoring service.

  • This type of service frequently sells for $79, or more, if purchased through other sources.
  • Joint accounts are eligible for a $30 discount for the second applicant, charged to their credit card.

Protect Your Cardssm
An application is included in the member packet or available online. Club members simply complete the application and return it to the address listed on the flyer. Remind people to keep the 800 number in a convenient location to report lost or stolen cards.

  • This service is convenient when cards are lost, stolen, when traveling, or when someone moves. Club members should be sure to take the 800 number with them when traveling.
  • Provides peace of mind for those with multiple cards.
  • The retail cost averages $15 or more per year if purchased through other sources.
  • Registration can include ATM and check cards.
  • Once information is entered, your customer will receive confirmation by mail that the registration is processed. They can call the 800 number listed on the flyer to verify if information is received by the administrator (please allow four to six weeks for input).
  • The administrators have highly secured facilities and procedures to assure customer confidentiality.
  • The service protects customers if they become liable for fraudulent charges.

Lost Key
An application is included in the member packet or available online. Simply complete the application and return it to the address listed on the flyer.

  • The club member will be enrolled in the Lost Key service and mailed a key tag to place on their key ring.
  • If keys are lost, the person finding the key tag places keys in a post office mail box. They are returned to the administrator who will then send keys to the individual registered with the key tag number.
  • Additional tags may be ordered for $3.95 each and paid for by check, money order, or cashier's check.

Protect Your Pet
An application is included in the member packet or available online. Simply complete the application and return it to the address listed on the flyer, or apply online.

  • An ID number will be assigned to the pet and will be on the tag when a lost pet is found. The administrator's 800 number can be called instead of the pet owner to keep your club member's name and location confidential. The administrators look up the pet by the ID number and contact your club member to recover their lost pet.
  • The pets name can be engraved on the ID tag.
  • Additional tags may be ordered for $3.95 each and paid for by check or credit card.

Travel Assistance Benefits

Discount Hotel/Motel Directory (Savings of up to 50% off the rack rate)
An application is included in the member packet or available online. Club members simply complete the application and return it to the address listed on the flyer. A directory and membership card will be mailed back to them. Remind people to read the directory's introductory pages to gain maximum value from this service. Some restrictions apply.

  • The average retail cost of similar services is $50 to $75 per year if purchased through other sources.
  • Has real dollar value savings for customers; no limitations for annual usage.

  • Easy to use -- simply call the 800 phone numbers for Alamo, Avis, Hertz, or National which are listed on the brochure found in the member packet. Be sure to give the appropriate ID number listed for each company.

800 Travel Service & Savings

  • A flyer is included in the member packet with their ID number to provide for reservations.
  • Club members simply call the 800 number on the flyer to be connected to Encore Travel Agency for full service travel needs.
  • Any commission-eligible travel ordered through Encore Travel allows your club member to receive a 5% refund by returning travel receipts, and the coupon they receive with travel documents. (Commission-eligible travel means airfare, rental cars and hotels/motels booked through this travel service.)
  • This service is totally independent of the Hotel/Motel Discount Directory.

Short Notice Travel Service

  • A flyer is included in the club member packet with their ID number to provide for reservations.
  • Customers call 301-731-5448 to listen to the most current recorded vacation descriptions. If interested, customers are provided with an 800 number to call at the end of the recording for making travel arrangements and reservations. Customer service representatives are available Monday - Friday, 9 a.m. - 9 p.m. ET, and Saturday from 9 a.m. - 5:30 p.m. ET.
  • The Short Notice reservationist will book the vacation over the phone and charge it to a credit card. Tickets, boarding passes and itineraries will either be sent to the customer directly or to the point of departure for pick-up.
  • This service appeals to customers who have the flexibility to travel.
  • Has real dollar value savings of hundreds of dollars.

Emergency Cash & Airline Tickets

  • A flyer is in the member packet to help provide convenience & peace of mind.
  • Club members should call 1-800-345-9988 to make arrangements to have up to $500 in cash wired to them depending on the terms & approval of their credit card issuer.
  • Call the same 800 number to arrange for prepaid airline tickets to be waiting at the nearest airport subject to the approval & terms of the credit card issuer.
  • Customers should be sure to take the 800 number with them when traveling.
24-Hour Emergency Roadside Assistance
  • A flyer with the toll-free number is included in the member packet.
  • The club member simply calls 800-863-0381 and receives assistance 24-hours a day at specially negotiated discount commercial rates on:
     Towing
     Jump starts
     Flat tire repair
     Lock-out assistance

  • The club member is responsible for payment at time of service.

PreScrip Prescription Program
An application is included in the member packet or available online. Club members simply complete the application and return it to the sponsoring organization as indicated on the flyer. Sisk will oversee card production and mail the cards directly to the sponsoring organization. It takes approximately 4 to 6 weeks for delivery.

  • Most major chains and independent pharmacies in all markets accept PreScrip.
  • Many people in the U.S. population are without a prescription card.

Emergency Medical Card
An application is included in the member packet or available online. Simply complete the application and return it to the address listed on the flyer. Important medical information can be provided to the card manufacturer who will reduce & transfer the medical information form to a laminated business card that can be easily carried at all times.

  • The Emergency Medical Card is flexible durable & about the size of a credit card, making it easily stored & carried in a purse or wallet. Emergency service personnel will have access to that important medical information in the event one becomes unconscious.
  • No personal or medical information is ever divulged for any other purpose nor is any information kept after cards are produced.
  • Cards can & should be updated yearly by providing the card manufacture with a new medical information form.

Preferred Health Card
An application is included in the Member Benefits packet and is available online.Simply complete the application and return it to the address indicated on the flyer. The club member will be mailed a packet of information and ID card good for all services except doctors and hospitals. An application for the separate ID card required for use at doctors and hospitals is included in the packet. There is no additional fee.

  • This service has a retail value of $199.95 and other similar services are over $350 per year.
  • This service has great website support for questions from bank employees, potential users or subscribers at www.preferredhealthcard.com.
  • Subscribers can easily find providers by entering the member ID number found on their ID card and their zip code.
  • This service is not an insurance plan but provides discounted fees for service.
  • All literature refers to savings up to 25% at participating doctors and hospitals, but it can be much greater as indicated on website examples.
  • Subscribers who order their doctor/hospital card will also receive a packet of user instructions and a letter to show participating doctors (in case the doctor’s office is not totally familiar with the plan).
  • Some doctors do not require pre-certification but fees are due at time of service. These are POS doctors.
  • An expanded list of doctors, specialists and hospitals do require pre-certification. Instructions for doing so are included in the subscriber’s membership materials.
  • Over 350,000 physicians and 70% of the nation’s hospitals accept our card.
  • This plan’s provider networks are called PPO Next and MD Select.

    The dental plan has approximately 15,000 providers in the DNOA network and Avesis, but additional dentists are available through the Cigna plan. The Cigna Plan requires the customer to request a separate ID card, but there is no additional fee.

Shopping and Entertainment

EasySaver
The core feature of this service is it allows club checking account members to access a website for great savings on various types of merchandise. The website address is www.myeasysaver.com and is found on the flyer in the Member Benefits packet along with the required ID number.

EasySaver also includes discounts at Meineke Car Centers and prescriptions. A member ID card will be needed to receive these point of sale discounts and can be ordered by completing the information on the fulfillment flyer and mailing it to the address indicated. The ID card also includes savings on rental cars at Alamo, Avis, Hertz and National as well as travel through Encore.

  • Savings amounts vary but can be up to 50% for brand name items like Sony, Black & Decker , Fisher Price, etc.
  • Savings exist up to 40% off for movie tickets at AMC and Crown Theaters and other discounts and theme parks.
  • Savings are up to 25% at Blockbuster or Hollywood Video chains.
  • Prepaid shopping and dining gift cards can be purchased from Home Depot, Red Lobster, Olive Garden, etc., which earn bonus gift cards.
  • If a user orders the ID card, an ID number will be printed on their card different from the generic number printed on our fulfillment flyer. Both numbers will work on the website where required in the member ID block. The user’s zip code will also need to be entered if they have ordered a card. Otherwise, enter the zip code found on the fulfillment flyer where asked for on the website.
  • Purchases made with the EasySaver program include extended warranty and guaranteed lowest price features. See website for details
  • EasySaver website purchases can be added to the users shopping cart and purchased with a credit/debit card. See the website for details.

Extended Warranty

  • A flyer outlining features and benefits is included in the member packet.
  • Most new retail items paid for with a check or debit card from the customers club account will have the manufacturer's warranty doubled up to one additional year for warranties of 5 years or less by calling.
  • No product registration is required.
  • To file a claim against a covered repair your club member will need to call 800-937-4639. They will be asked to provide:
     Copy of the canceled check used to purchase the item.
     Copy of the store receipt.
     Copy of the manufactures warranty, the warranty card and product serial number.
  • Some items are excluded -- read the flyer in the member packet.
  • Each account has a $1,000 limit per occurrence and a $50,000 annual payment limit.

90 Day Casualty Protection

  • A flyer outlining features and benefits is included in the member packet.
  • Most new consumer products paid for with a check or debit card from the club account will be covered against direct physical loss or damage for 90 days from date of purchase.
  • This coverage is secondary to other coverage's such as homeowners or renters policies.
  • No product registration is required.
  • To file a claim, call the administrators at 1-800-937-4639 within 45 days of the loss to request a loss or damage form. Complete and return it and other requested information within 90 days of the loss.
  • Items repairable will be fixed at a network of over 6000 repair facilities.
  • Some exclusions exist.
  • Club members should read the flyer in their packet.

Price Plussm Price Protection

  • A flyer outlining features and benefits is included in the member packet.
  • After making a qualifying purchase with a check or debit card from the club account, if the club member finds the identical product in a printed ad for a lower price, this service allows them to receive a refund for the difference in price.
  • The service applies to products only, no services.
  • Most new items are covered if purchased in the U.S. from an authorized dealer. Some items are excluded.
  • To file a claim call 1-800-937-4639 24 hours a day within 10 days of seeing the printed advertisement.
     Request a refund form.
     Within 30 days of requesting the form, complete and return it and the original canceled check, the original store receipt & the original printed advertisement listing the identical product.
  • Club members should read the flyer in their packet.

Entertainment Discounts
An application is included in the member packet or available online. Simply complete the application and return it to the address listed on the flyer.
